How does someone adopt a New Leash on Life dog?

Fill out a humane society adoption application that has vet and 2 personal references. Then we do a home visit, with a 2 week trial period. If there is a behavior problem that they are concerned about, the Humane Society will pay for one visit with a professional trainer who will diagnose and attempt to help the family solve the problem. The adoption fee is $90.
